How to Refer
Referring is simple. Please follow the steps below to ensure your referral is processed smoothly:
Complete the Referral Form
Fill out all required fields to provide us with information about the person you are referring.
Submit Supporting Documentation
If available, attach any relevant documents that will help us understand the needs and goals of the individual.
Consent
Ensure you have the consent of the individual being referred to share their information with us for the purposes of obtaining NDIS support services.
Submit the Form
Once you have completed the form and gathered any necessary documentation, email it to us at info@disabilitysupportnsw.com. We will process your referral and get in touch with you and the referred individual as soon as possible.
